Website Terms of Use and Medical Disclaimer

Dolomite Child & Adult Psychiatry, PLLC
Effective October 8, 2026

General information and care

This website provides general information about the practice and its services. It does not provide individualized diagnosis, treatment, prescribing, or emergency assistance. Do not start, stop, or change medication based on website content. Discuss personal medical questions with an appropriate clinician. Browsing the site, submitting a form, or receiving an administrative reply does not by itself establish a treatment relationship or guarantee that the practice can accept you as a patient. Care begins only after the practice accepts the patient and completes its clinical and consent process. Video telehealth visits use Zoom. Outpatient services are provided only when the patient is physically in Illinois under the practice's current service policy. Availability, suitability for telehealth, insurance participation, and prescribing decisions depend on the individual circumstances and applicable requirements. No specific clinical result or prescription is promised.

Emergencies and communications

Dolomite is not a crisis, emergency, or on-call service and does not provide 24/7 coverage. Website forms, patient portals, email, text, and voicemail are not continuously monitored. Call 911 or go to the nearest emergency department for an immediate medical or safety emergency. For the Suicide & Crisis Lifeline, call or text 988; 988 does not replace 911 for an immediate emergency. Use approved patient channels for confidential clinical information. Submission of a website inquiry is not consent to office texting, AI recording, or disclosure of records. Those choices require separate consent processes. Office SMS uses the documented verbal opt-in described below; AI recording and records disclosures require their applicable separate permissions.

Responsible use and outside links

Use the website lawfully. Do not try to access another person's information, interfere with the service, upload harmful software, or impersonate someone. Do not send payment card or identity documents through an ordinary contact form. The practice may limit misuse of website functions while respecting patient rights and continuity obligations. Outside links are provided for convenience. Their inclusion does not guarantee their accuracy or endorse all content. Website information may change, and temporary errors or outages may occur. Contact the office to verify scheduling, fees, coverage, and service availability.

Relationship to patient documents

These terms do not waive privacy rights, informed consent rights, or professional duties. Clinical consent, financial terms, the Notice of Privacy Practices, and separately signed authorizations govern their respective subjects. The SMS Terms and Conditions below govern the optional office SMS program and are also available at SMS Terms and Conditions. Updates to website terms do not retroactively expand an existing patient authorization.

Message and data rates may apply according to your mobile carrier’s plan. Charges may vary for domestic or international messaging. Contact your carrier for pricing details.

5. Opt-In Method

You may opt in verbally during a telephone or in-person conversation with our staff. Before asking for your permission, staff explain the appointment-related messages, variable frequency, possible message and data rates, STOP and HELP instructions, and where to find our Privacy Policy and SMS Terms and Conditions. Staff ask for your affirmative agreement to receive these texts at the mobile number you identify and document your response.

Texting is optional and is not a condition of care. Providing a phone number, submitting an inquiry, signing treatment paperwork, or selecting a preference on an intake form does not by itself enroll you in this verbal-consent SMS program.

6. Opt-Out Method

Reply STOP to any office text to opt out at any time. You may also call (312) 426-2705, email info@dolomitepsych.com, or tell our staff directly that you want texts to stop. We honor reasonable opt-out requests and may send one non-promotional confirmation. Stopping texts does not cancel appointments or affect access to care. Contact us to arrange another communication method. We will not restart texts without a new documented opt-in.

7. Help

Reply HELP for assistance, call (312) 426-2705, email info@dolomitepsych.com, or visit www.dolomitepsychiatry.com.

8. Standard Messaging Disclosures

Message and data rates may apply. Message frequency varies. Reply STOP to opt out or HELP for assistance. Read our Privacy Policy, SMS Terms and Conditions, and Website Terms of Use.

Ordinary SMS is not a secure patient portal. Messages may reveal the practice name or appointment information to someone with access to your device or account. Do not send symptoms, diagnoses, medication details, records, payment card details, or other sensitive information by ordinary SMS. Use the approved patient portal or call for a secure communication method. Tell us if your number changes, is shared, or belongs to someone else.

Delivery is not guaranteed. Mobile carriers are not liable for delayed or undelivered messages. Texts are not continuously monitored and are not for urgent needs. Dolomite is not an emergency or on-call service. For an immediate medical or safety emergency, call 911 or go to the nearest emergency department.
